Community to Home (CTH) is a free
home-visiting program offered across
Pennsylvania to Children and Youth with
Special Healthcare Needs (CYSHCN) and
their families residing in rural counties.
CTH aims to improve health outcomes
for CYSHCN who are impacted by health
disparities. CTH utilizes evidence-based
Community Health Worker (CHW) models
to diminish barriers and promote equal
quality healthcare access.
Community to Home services will be provided
by CareStar and Health Promotion Council
depending on the family’s county of residence.
